4/23/2023 EnchantaVaughn Approved Behavior: Upon arrival to the home setting, Ben-Nita appeared to be in a positive mood. Ben-Nita described that she worries about issues related to personal safety and her mental health, among other things. Ben-Nita reports having issues in school with her peers. Behavior: Ben-Nita expressed resentment for not being able to be open with her mom during this difficult moment in time. Ben-Nita continued to show guarded and distant behaviors. Ben-Nita admitted that some of her decisions can cause her to hurt other people close to her. Intervention: QMHP inquired about Ben-Nita’s day in the home and what things she’s doing to make it a good day. QMHP educated Ben-Nita about what she can do to make her daily activities more fun and interesting, such as reading her favorite book, drawing in a journal, stretching and closing her eyes and practicing the mindful behaviors learned in sessions. QMHP offered active listening skills when Ben-Nita wanted to speak. Intervention: QMHP led the Ben-Nita through each therapeutic day treatment exercise. QMHP motivated Ben-Nita to respond to exercise and express her feelings. QMHP modeled proper positive behavioral expectations for the Ben-Nita to encourage additional layers of support and understanding of rules and expectations. QMHP processed with Ben-Nita on what she needs to do to achieve ISP objectives. QMHP encouraged Ben-Nita to be the example for others. QMHP encouraged Ben-Nita not to follow others she sees that are doing wrong. QMHP commended Ben-Nita on her efforts to maintain her ISP objectives as best as she could even with minor difficulties. QMHP observed Ben-Nita preparing for the rest of the evening in a positive manner. QMHP will implement a routine that will encourage Ben-Nita to be responsible and promote self-confidence in academic and social settings. Response: Ben-Nita displays normal behavior in therapeutic setting. Ben-Nita exhibited a positive attitude toward mom and counselor at the beginning of the session. Ben-Nita has implemented the correct behaviors in session toward in-home staff. Ben-Nita was observed avoiding disruptive and impulsive actions demonstrated when Ben-Nita proceeded to walk away and utilize deep breathing. Ben-Nita was observed intervening and regulating her emotions. Ben-Nita knows what is expected of her and is trying to make better choices moving forward. Response: Ben-Nita was observed in her seat and raising her hand when she wanted to ask a question. Ben-Nita interacted well in the session and demonstrates good work ethic. Ben-Nita finds more excitement in learning about her mental illness. Ben-Nita was fully engaged in the activities presented to her today, showing great sportsmanship and responsibilities in the home. Ben-Nita displays good attitude by saying “thank you”.
